Services and Care
Personal Care
We provide needed assistance with activities of daily living and supervision. The center will assist with ADL's as set forth in a personalized care plan. Including toileting, feeding, and transferring.
Nursing Services
Our licensed nurse is onsite during hours of operation, to provide treatment, health screenings, health education, medication administration and other care with valid physician orders.
Social Services
We assist clients and their care givers as needed to
enhance the effectiveness of the client’s care plan.
Transportation
Holly Adult Day Center does not provide transportation but can assist with LogistiCare and DART paratransit signup. Families are encouraged to transport their loved one.
Therapeutic Activities
We provide a structured program designed to be fun, and promote personal growth to improve or maintain the level of functioning for the client. A planned schedule of recreational, motivational, social, and therapeutic activities.
Nutritious Meals
We provide nutritious meals that are appropriate based on each client’s clinical need. Includes breakfast, lunch, and an afternoon snack. We follow CACFP nutritional guidelines. Sherm's will be catering lunch.
Additional Services
Physical Therapy, Occupational Therapy,
Speech Therapy, and Podiatry can be arranged dependent on client's insurance. Cost is not included in daily rate; the client's insurance company will be billed by the provider for these services.
Center will provide showers for an additional rate.
